The Brooklyn Hospital Center's Home Health Services staff is capable of managing complex cases, many from elderly, underserved, and multicultural populations. Caring for the people of Brooklyn is our specialty.

Our vision is to provide comprehensive patient care that smoothes the transition from hospital to home. We offer a wide range of services in the home so patients can recuperate in greater comfort, close to family and friends.

Home Health Services provides compassionate home care in coordination with the patient’s physician. The patient does not have to be hospitalized to qualify. In fact, home care often represents a preferable alternative to hospitalization for patients who need intermittent, skilled nursing or rehabilitative care.
